Valley Health System, in collaboration with Inbound Health, is set to launch a Hospital at Home programme this summer, delivering hospital-level care to patients in their own homes.

The initiative aims to facilitate healing in a familiar environment, enhancing patient experience while ensuring the same quality and safety standards as The Valley Hospital.

Valley’s Hospital at Home programme offers eligible patients the same high-quality care available at The Valley Hospital, within the comfort of their homes.

It requires patients to initially visit the Valley Hospital for an assessment of their medical condition and home safety protocols, and eligible patients will be referred to the programme.

Valley Health System president and CEO Robert Brenner said: “Valley is pleased to partner with Inbound Health to bring hospital-level care to our patients where they feel most comfortable – at home.

Patients enrolled in the programme receive daily clinical assessments, 24/7 monitoring, medical supplies, and home visits from nurses and clinicians for several conditions.

The conditions include pneumonia, congestive heart failure, or cellulitis.

Inbound Health collaborates with hospitals to transform care delivery by extending high-acuity care into homes.

The company offers expertise in launching and scaling advanced care-at-home models to meet clinical, operational, and financial objectives.

The partnership leverages Inbound’s virtual and in-market care teams, technology, logistics, and clinical protocols to deliver safe, scalable, and effective care outside the hospital setting.
