
Medsien has unveiled its Advanced Primary Care Management (APCM) programme to support healthcare organisations in the adoption of the new care model introduced by the Centres for Medicare & Medicaid Services (CMS).
The initiative seeks to transform the delivery of chronic care and facilitate sustainable growth for healthcare providers.
The APCM model, set to be implemented by CMS in 2025, introduces a significant shift in Medicare reimbursement strategies.
It replaces traditional time-based requirements with a monthly per-patient payment system.
The model utilises billing codes G0556, G0557, and G0558, allowing providers to categorise patients by risk and receive bundled payments for various services.
The services encompass chronic care management (CCM), transitional care management (TCM), primary care management (PCM), virtual visits, telehealth check-ins, and remote evaluations.
Medsien said the focus is on rewarding readiness and continuity of care, rather than administrative time tracking, enabling practices to prioritise patient care.
Medsien’s APCM programme is designed to implement this vision rapidly, with practices able to go live within five days.
The programme offers a turnkey implementation that negates the need for additional staff or complex onboarding processes. It integrates seamlessly with Electronic Health Records (EHR) for consistent and automated documentation.
AI-driven workflows are employed to streamline operations and ensure compliance. The programme also includes a full-service delivery model, covering everything from patient identification to ongoing, personalised outreach. Care delivery is managed by Medsien’s team of experienced care partners.
