
US-based virtual healthcare services provider Teladoc Health has launched its next-generation Cardiometabolic Health Programme to improve population health.
The programme aims to enhance population health and prevent the progression of diabetes, hypertension, and obesity.
It focuses on optimising cardiometabolic health by addressing key lifestyle factors such as diet, physical activity, sleep, stress management, and weight.
The initiative aligns with the American Heart Association’s Life’s Essential 8 measures.
It offers a premium subscription to the BetterSleep app, registered dietitians for personalised nutrition planning, and expert health coaches for high-risk individuals.
The program also offers at-home testing for cardiometabolic measures like A1c and blood lipids, and dynamic health insights leveraging patient-reported data and connected devices.
Teladoc Health US group health president Kelly Bliss said: “Nutrition, activity, sleep and stress management all have an outsized impact on population health.
“Our program comprehensively addresses these foundational pillars of cardiometabolic health because we know that’s what it takes to deliver meaningful outcomes, both for those managing multiple conditions and those looking to avoid progression of the disease.
“Conditions and their risk factors are often interconnected, and with the right tools and behaviour changes, they can be prevented.”
Teladoc Health’s Cardiometabolic Health Programme easily integrates with its 24/7 Care and primary care providers, along with connected care partners for related conditions.
The initiative builds on the company’s successful chronic condition management programmes, which improved diabetes and hypertension management among participants.
According to recent data, 58% of participants in Teladoc’s diabetes management programme achieved remission-level A1c, and 88% in the hypertension programme improved or maintained their blood pressure after one year.
The new Cardiometabolic Health Programme is part of Teladoc Health’s strategy to enhance healthcare outcomes through its integrated care segment.
Recently, the company expanded its at-home testing and preventative care capabilities, along with weight management programmes, and formed new collaborations.
Earlier this year, Teladoc Health agreed to acquire Catapult Health, which provides Virtual Preventive Care, Virtual Primary Care and VirtualCheckup services, for $65m.
