Guy’s and St Thomas’ NHS Foundation Trust has unveiled the Proactive & Accessible Transformation of Healthcare (PATH) initiative, partnering with General Catalyst, NVIDIA, Hippocratic AI, and Sword Health.

The collaboration aims to advance the NHS’s strategic goals by shifting healthcare delivery from hospitals to communities, transitioning from analogue to digital systems, and focusing on prevention rather than treatment.

PATH seeks to leverage the latest machine learning and AI technologies to enhance NHS care delivery, aiming for a more proactive and efficient health system and establishing a scalable national model for population health.

The initiative will initially address the pressing issue of elective care waiting lists at Guy’s and St Thomas’. Currently, over 53,000 patients are awaiting their first appointment, and 25,000 are in line for surgery.

PATH intends to overhaul the patient journey by integrating AI-driven patient support, remote monitoring, and virtual care solutions.

This approach will enable clinicians to prioritise based on clinical needs, empower patients to actively participate in their care, and ensure efficient resource allocation. The goal is to create a responsive healthcare model that maintains timely access and quality care.

PATH combines several key components: Guy’s and St Thomas’ Centre of Innovation, Transformation, and Improvement (CITI), Hippocratic AI’s generative AI healthcare agents, Sword Health’s AI Care platform, NVIDIA’s Inception program for startups, and General Catalyst’s health system expertise.

Hippocratic AI has already conducted over 2.49 million patient calls with a high satisfaction rating, while Sword Health has treated over 500,000 patients globally, delivering 6.5 million AI sessions and saving nearly $1bn in unnecessary costs.

NVIDIA provides technical tools and training for AI developers, while General Catalyst identifies transformative technology providers and aligns incentives through shared-savings models.
